Next Board Meeting
August 26th, 2010
The August board meeting will be held at the Lincoln County Ambulance District's Troy Office at 6:30 PM. All further meetings will be held the 3rd Thursday of the month. The address for the Ambulance base is 1392 S. 3rd Street.
Follow the steps below to complete the online registration process.

Print the forms and complete them with the required information.

Scan a copy of the form and save it with a file name using the following format:
Number 2-7 indicating age of team that the child will be playing for, an underscore, the childs last name, an underscore, and indicate either football or cheerleading. An example using the signup document for a child playing for the second grade team and the last name of Smith would be 2_Smith_football.xxx with xxx being the file extension. The preferred method would be to save the scans into an Adobe PDF file or add all of the scans to a zip file.
